| Q. | What is the subscription cost for a Medem Web site and iHealth services? |
| A. | The annual subscription fee is $395/physician per year; extremely modest when compared to other Web hosting and Internet services. Approximately $1 per day secures a robust offering of tools that benefit your practice and patients, including a web site, secure messaging, an industry leading Personal Health Record, award winning medical library and adherence programs. Discounts are available to members of participating medical societies and trusted partners. |

I am a partner society member, but I believe I'm being charged at the non-society rate, what should I do? |
| A. | Please fax a copy of your society ID card with your contact information to (877) 526-3356 and a Medem representative will contact you to update your record. |

What services are included in my subscription? |

The following iHealth services are included in the cost of your annual subscription:
Full-feature customizable Web site including video, maps, Medem library patient education materials, pre-registration forms and much more.
iHealth Web sites also feature:
Automated daily updates and alerts sent directly from the FDA bring peace of mind to your patients and help reduce your liability. Instead of learning about adverse drug interactions or recalls from the news, your patients immediately and automatically receive FDA warnings and notifications. This patient safety feature is just one of the many reasons that leading medical liability carriers endorse iHealth. Disease Management and Medication Adherence Programs When patients complete an iHealthRecord, they are automatically enrolled in appropriate disease management and medication adherence programs from leading medical societies, patient advocacy groups, and government agencies (including the FDA, CDC, American Heart Association, American Cancer Society, etc). You may also create customized adherence programs to suit patients' needs. |

| A. | Subscription nonpayment will result in the automatic termination of your practice Web site and prevent access to iHealth, depriving your patients the benefit of these services. |

| A. | While we are sorry to lose any of our valued customers, please remember that unpaid bills will result in automatic service termination. Medem does not send unpaid subscriptions to collection. What do I need to do to cancel my subscription? |

Please provide Medem with your invoice number, account number, Web address and first and last names. Although we are sorry to lose any of our valued customers, and encourage you to reconsider, Medem processes cancellation requests immediately.
